In-House Dental Plan Membership
How It Works
​Our “In-House Dental Plan” is our own service/discount program. It is
designed for patients who plan to pay out of pocket expenses and do not have dental insurance coverage. All services under the plan are provided within our offices (Portland, Maine and Jonesport, Maine). It is
not a commercial dental insurance program. *
Not currently in network with insurance
Our office does not currently accept dental insurance but we will issue dental claim (ADA) forms for a self-submitting reimbursement by your insurance company.
We are not responsible, nor can we predict what insurance is willing to reimburse for services. We can suggest that you refer to your insurance policy admin for more details. If you have further questions about submitting to your insurance, we are happy to answer any questions.
What is covered under the plan?
(Periodontal plan option covers all recommended perio maintenance)
